Forms
Before we can work on your gums and teeth, there are some things we need to know and some things the state needs you to sign. You can complete your new patient forms online before your visit, print and fill them out at home, or simply arrive 15 minutes early and our New Patient Coordinator will be happy to assist you in the office.
What to Bring?
Please bring your ID, Insurance Card, any x-rays or records you may have from your previous dentist.
X-Rays
Your hygienist or assistant will greet you, get to know you, and then take x-rays of your mouth. We need these x-rays so our dentists can get a comprehensive look at your oral health. We utilize the latest technology in digital x-rays, so they are safe and effective. Please feel free to bring any current x-rays to the office.
Cleaning
If you are not here for a dental emergency, our hygienist will perform a thorough teeth cleaning. We will discuss your medical history and any concerns you may have.
Dr. Exam
After your thorough dental cleaning, our doctor and his assistant will meet you. He will do a thorough examination of each tooth, gums, and review all x-rays with you. After answering questions or concerns you may have, he will make his own recommendations of further treatment if needed.
